Does my loved one qualify for services?
Qualifying diagnosis:
Any type of intellectual, developmental, or physical disability may qualify a person for habilitation services. Examples include autism spectrum disorder, intellectual or developmental disabilities, and cerebral palsy.
Age:
Most BAYADA Habilitation programs are designed for adults aged 18 and up.
Geographic location:
Your loved one must reside within an area in which BAYADA Habilitation serves.
Referral:
If using insurance to pay for services, a referral from a primary care physician is typically required. Case managers can also initiate the process under state-sponsored Home and Community-Based Services waiver programs.
Insurance:
Our services are typically paid for through insurance, including Medicaid, as well as Home and Community-Based Services waivers. Private pay is also accepted.
What is the process for getting services?
Contact BAYADA:
The process begins when a client, family member, referral source, or provider calls their local BAYADA Habilitation office about services. We’ll collect essential information—such as diagnosis, skills, client and family details, referral source—to confirm eligibility for services.
Complete client consent forms:
The client, family member, or guardian signs necessary consent forms so we can begin the assessment process.
Assessment visit:
A BAYADA Habilitation professional conducts a home or facility visit to thoroughly evaluate your loved one’s needs, living environment, and suitability for habilitation services.
Personalized care plan:
Using the assessment findings, we design a plan best tailored to your loved one’s goals to improve life skills and promote independence.
Start services:
We take care in matching your loved one with a caregiver (habilitation technician) who is a good fit and schedule the first visit.
Ongoing support:
The office team will work with you and your loved one to create a schedule of regular visits, and will provide ongoing management of services. Should needs and goals change over time, we will make appropriate changes to the care plan. We are dedicated to ensuring a positive and productive experience for you and your loved one.
What are the payment options?
BAYADA Habilitation accepts reimbursement from a broad range of payor sources.
Medicaid (including waivers such as the Innovation Waiver, 1915i, and B3 services, depending on state programs)
Managed Medicaid

Government programs and contracts (varies by state)
School district contracts
Private pay:
If you don’t have insurance for our services—or if you seek additional services that your plan doesn’t cover—you may opt to pay out-of-pocket for habilitation services. If so, we can work out a payment plan with you.
