Before we visit your parents’ home an initial assessment is performed. One of Korea Home Health Care clinically trained team members, in conjunction with immediate family and the treating physician, will use this assessment to determine the best plan of care for your parent(s) or loved one(s). This plan of care will outline the specific services that will be provided.

Our qualified team will then determine which care provider is best suited to your love one's unique needs, assign care providers, and schedule an appointment for an initial home visit. If the client has special requirements, the client coordinator assigned to their case will include this in documentation for future visits.

Once a care plan has been outlined and a clinician assigned to your parents, the nurse assigned to your loved one(s) will visit their home and conduct a detailed health assessment to identify any significant changes in health or physical condition. The visits will be as frequent as the care plan requires.

Korea Home Health Care employs a team approach where clinicians and caregivers work together to care for our clients. Their work is overseen by a clinical manager to ensure that all care is high quality and in alignment with the client’s doctor’s guidance.

The nurse assigned to you parents’ care can:
