Home health care may support people recovering from an illness, injury, surgery, or hospital stay. It can also support people managing chronic or complex health needs at home. 
Eligibility depends on the type of service, clinical need, location, and insurance requirements. Some skilled services require an order from a qualified health care provider. 

Medicare may cover eligible home health services, including skilled nursing and therapy.
To qualify, a person generally must be homebound, need part-time or intermittent skilled care, and receive services ordered by a qualified provider. Medicare generally does not cover personal care by itself, such as help with bathing, dressing, or other daily activities.

Home health care brings nursing, therapy, and other coordinated support into a person’s home.
Depending on the care plan, teams may reinforce medication routines, monitor changes, and communicate concerns to physicians, pharmacists, and family caregivers. The goal is to support recovery and help people remain safe and independent at home.
